(full name) as his/her proxy, to represent the undersigned at the Area meetings on Thursday, Nov. 19, 2009 between 9:00 am and 2:00 pm; and at the Annual Meeting of the members of the corporation to be held on Thursday, November 19, 200 at approximately 3:30 p.m., or at any adjournment thereof and to vote for the undersigned on any and all matters that may come before the meetings.
